The Transition Age Youth (TAY) Division oversees a wide range of services and programs for young adults ages 18-26 in Delaware County. These services and programs are delivered by a pool of provider agencies which have many years of experience working with Transition Age Youth.
The TAY division assures appropriate services are available for TAY ages 18-26 who are experiencing mental health challenges.
They contract with agencies who provide an array of services and resources which are focused around trauma informed principles, recovery, resilience and self-determination. A few of the many services offered are High Fidelity Wraparound, Outpatient Treatment, Blended Case Management, Transition Age Housing, and Program of Assertive Community Treatment.
